For a fallout-style story, what genre would it belong to? Modern? Sci-fi? Horror?

Sci-fi, unless it has a very strong horror element or something.

Okay, let me rephrase that.

For a story set after a nuclear war similar to Fallout, but without robots or plasma guns or lasers, but still with mutants, would it still be considered sci-fi?

I’d still put it under scif, but like Steve said, if the tone is really giving off a more horror vibe, like you’re doing more running and hiding from invincible mutants as opposed to trading with them or easily shooting them, then horror might be a better fit.

I'd assume the system would go like this: Fanfiction, Theme, Setting. Obviously, if it's a Fanfiction, that's the genre no matter what. If it's not, it would go to something like Horror, Puzzle or Love, so being hunted by Fantasy Vampires, Werewolves and Demons or genetically modified aliens still falls under those categories. If it's not one of those, then it'd fit in with where the setting is: Fantasy World, Sci-Fi horror (Which includes post-apocalyse), or modern day.
